Having just received your new orthosis or prosthesis, you may have a few questions regarding the wear and care of this device. Please review the information below and ask your practitioner if you have any further questions.
PRE-APPOINTMENT CHECKLIST
If you are a new patient or if it has been over a year since your last appointment with Streamline, you will need to update all of your records. You will also be asked to provide insurance and medical history information.

MEDICARE CHART NOTES
To be covered by Medicare, all applicable Medicare statutory and regulatory requirements must be met. Medicare requires a physical evaluation of the patient.
